I, [ {name} ], hereby acknowledge that The participation in martial arts training sessions at 9 Points Muay Thai & MMA/ Cabot Jiujitsu and Fitness (later referred to as The Dojo) located in Cabot, Arkansas, or any location whereas I have gone to compete, observe or train with under The Dojo guidance, involves certain inherent risks, including but not limited to the risk of injury, death, or dismemberment. In consideration of being permitted to participate in these activities, I voluntarily agree to assume all risks associated with such participation.

I understand and acknowledge that martial arts training involves physical contact, strenuous physical activity, and the use of various training equipment. I acknowledge that the practice of martial arts includes contact with other participants and instructors, which may result in accidental injury, including but not limited to bruises, strains, sprains, fractures, or more severe injuries.

I further acknowledge that The Dojo, its property owners, instructors, staff members, and other participants cannot guarantee my safety or prevent injuries that may occur during martial arts training sessions.

I agree to release, waive, and discharge The Dojo, its property owners, instructors, staff members, and other participants from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation in martial arts training sessions.

I acknowledge and agree that I am solely responsible for my actions and my physical condition during training sessions. I understand the importance of following instructions provided by the instructors and adhering to safety guidelines to minimize the risk of injury.

I understand that this waiver of liability covers liability arising from ordinary negligence but does not cover liability arising from gross negligence or intentional misconduct.

I hereby waive any right to bring legal action or assert a claim against The Dojo, its property owners, instructors, staff members, and other participants for any damages, injuries, or criminal charges that may occur during or as a result of my participation in martial arts training sessions.

I have read this waiver and fully understand its contents. I acknowledge that I am signing this document voluntarily and of my own free will.
